The Healthy Initiatives HealthPlan encompasses wellness, medical management, disease management, and a healthy lifestyle maintenance program. Many of the leading causes of death and illness can be effectively managed through early detection, proper medical treatment, pharmacological management, and behavioral intervention and support. The program includes age and gender based biometric screenings, health risk assessment and an incentive program for Healthy Initiatives compliance. The biometric screenings will be paid at 100% of the network fee or at the reasonable and customary rate for non-network providers and do not accumulate towards the preventative/routine calendar year maximum.

The TML IEBP Medical Benefit Book contains the community-based screenings that are included in the Healthy Initiatives HealthPlan. The program is designed to encourage proactive healthcare identification, intervention and healthy initiative lifestyle maintenance. The program supports the interface of medical, prescription and health risk assessment information. The information is stratified so that the Interdisciplinary Medical Management Team can identify the covered individuals that require a customized Healthy Initiatives Welcome Kit and/or a Personal Health Coach.

The Healthy Initiatives HealthPlan works with the covered individual to identify solutions to healthcare concerns. TML IEBP’s Healthy Initiative HealthPlan should assist in patient care compliance and healthcare initiative from your employee and dependent population. TML IEBP will implement integrated data and reporting along with customized supportive programs that help plan members achieve behavior changes and long-term improved health.

TML IEBP’s Interdisciplinary Medical Management Team work with covered individuals and educate them on healthy lifestyle behavior, cost effective medication options, physician visit preparation, patient’s rights, understanding the provider network options and choices, and becoming an educated consumer for healthcare services. In addition, TML IEBP provides access to wellness information through their website links, provides benefit support for community-based biometric wellness screenings, healthcare services, educational programs and/or health fairs, and preventive education workshops.
